REFERRAL PROCESS

Client referrals are made by submitting a form (see below) via email. Agencies can also contact our office directly to discuss accommodation and support needs.

We accept referrals from various agencies, such as, but not limited to, Social Services, Probation Service, Youth Offending Teams, Benefits Agencies, Housing Departments and other voluntary & statutory organisations.

All referrals must be accompanied by a completed risk assessment and/or supporting documents (where possible). Referrals are then assessed for their eligibility for supported accommodation. Applicants will then undergo a screening process and needs assessment to determine their support needs and establish whether these can be effectively met by our service.

After a comprehensive assessment of the applicants needs, a decision will be made as to whether we can offer appropriate supported accommodation, and the referral agency will be informed of this decision.

If the applicant is offered supported accommodation, they will then be matched with a Support Worker and given a support package designed to meet their individual needs.
